I, ______________________________________ parent(s) of ____________________________________ accept full responsibility for all uniforms and instruments utilized for my student's participation in the band organization. I also understand that all monies paid for fees, products, trips, dues, deposits or fundraisers are non-refundable.

In regards to any fundraising activities, it is also my responsibility to adhere to all time lines established by the organization for pick-up and drop off of products or materials. I will not hold the band responsible for my negligence.

PARENT'S PLEDGE

As the parent(s) of ______________________________ I commit to attend a minimum of 3 performances this band season. I will make an hones effort to communicate with Ms Sharon Allen or Mrs. Carla Butler in regards to dates, attire, performance and payments. I will teach my young adult the true meaning of commitment by displaying that same commitment to him/her, the band and the parent organization.
